Our Services

At Hands of Hope Adult Medical Day Services, Inc., we offer a wide range of services designed to support the medical, social, and emotional well-being of our participants. Our comprehensive daytime program promotes health, independence, stimulation, and community engagement in a safe and nurturing environment.

On-Site Nursing Care & Health Monitoring

A Registered Nurse is on duty each day to supervise all medical services. Our nursing team conducts health assessments, monitors vital signs, manages medications, and coordinates care with participants’ physicians. We administer medications, provide basic treatments, and respond promptly to health changes or emergencies in accordance with each individual’s care plan and Maryland regulations.

Personal Care Assistance

Our trained Certified Nursing Assistants (CNAs) and program aides assist participants with activities of daily living, including bathing, grooming, toileting, mobility, and eating as needed. We provide personal care with dignity, compassion, and respect. With a staff-to-participant ratio of at least 1:7, we ensure close supervision and individualized attention throughout the day. 

Therapeutic Activities Program

Each day features engaging activities designed to promote physical movement, cognitive stimulation, and social interaction. Our full-time Activities Coordinator offers programs such as:

Activities are tailored to participants’ abilities and interests, with modifications provided to encourage full participation. We also organize occasional supervised community outings with appropriate staffing support.

Engaging Activities & Person-Centered Programming

Participants remain active and engaged through creative daily programming. Our staff guide seniors through art, games, music, and gentle exercises designed to promote movement, memory, and enjoyment.

We emphasize person-centered planning—each participant is assessed upon enrollment to identify recreational interests, strengths, and preferences. Activities are selected to be meaningful and enjoyable, whether it’s a morning stretch group, a sing-along of favorite oldies, or individualized sensory stimulation. Participation is always encouraged but never forced, and both group and one-on-one options are available to accommodate individual comfort levels.

Nutritious Meals & Snacks

We provide healthy, balanced meals to support overall wellness. Participants receive a hot lunch daily, along with morning and/or afternoon snacks. Menus are designed to meet at least one-third of daily nutritional requirements and are reviewed by a dietetic professional.

Meals are served in a warm, social dining environment, with staff assistance provided as needed. Special meals, themed lunches, birthdays, and holiday celebrations are incorporated to make mealtimes enjoyable and engaging.

Social Work Services

Emotional and social well-being are essential to quality care. A Licensed Social Worker is part of our interdisciplinary team and provides ongoing psychosocial support to participants and families.

Our social workers serve as an advocate and connector, helping families access community resources, respite services, and emotional support when needed.

Transportation Services

Safe and reliable transportation is available for participants who need it. We offer door-to-door, wheelchair-accessible van transportation, provided by trained staff who ensure comfort and safety at all times.

Transportation may be covered for participants enrolled through Maryland Medicaid waiver programs. Private-pay transportation is also available for an additional fee.

Care Coordination

Our team works collaboratively with families, physicians, and outside providers to ensure seamless care. Regular care plan reviews are conducted—typically every six months or as needed—by our interdisciplinary team, which includes nursing, social work, activities, and program leadership.

Families receive regular updates, and any changes in health or behavior are promptly communicated. Our coordinated approach ensures comprehensive, well-rounded support for every participant.

Family & Caregiver Support

At Hands of Hope, we view families as valued partners in care. We offer caregiver education, emotional support, and ongoing communication to help ease the challenges of caregiving.
